Ensure the individual meets Medi-Cal and ALW program criteria. This typically involves a needs assessment by a Care Coordination Agency (CCA).
Work with a participating Care Coordination Agency to complete all required assessments and documentation for ALW approval.
Once approved, your CCA will coordinate with Pine House Villa to confirm current bed availability and facilitate placement.
